      Contact the church office and set up a date for baptism with one of the pastors - 743.6218. Once a date has been confirmed please complete the Baptism Information Form and return it to the church office.

      Building Use Request

      Anyone wishing to use the facilities of the Sturgeon Bay Moravian Church must first complete a building use request form and submit it to the church office for review. One time use issues can be cleared through the church office. Ongoing facility use, by a group or organization, need to be cleared by the Board of Elders which meet the second Monday of the month.

      Discipleship (Confirmation) Participant Form

      Discipleship classes start in October and run through April.  Please contact the church office and notify them of your desire to join a class.  We also ask that you return a completed Discipleship form to the church office.

      Graduate Sunday

      This is a generic form to register you or your student for recognition on Graduate Sunday which takes place the last Sunday in May or first Sunday in June.  Please insert the appropriate date where indicated - if you know the date of Graduate Sunday please use that date.  If you are uncertain of the Graduate Sunday date please put in the current year so that we are sure we are using current information.

      Memorial (Funeral) Planning

      No on likes to think about end of life issues, but it is a reality of life.  We, at the Moravian Church, want the service for you or your loved one to be a true celebration of life.  We strive to make the memorial service a time to praise God for this life and also a time to reflect on the personal faith journey of the individual.  We have attached a link to a planner that will help the pastor make the memorial service a more personal testimony.  Complete the form and provide copies for your family, put a copy in a safe place and give a copy to the church to file away for you.  For a more complete list of memorial planning information stop in or contact the church office for a copy of the Memorial Planner Booklet.

      ALL SAINTS’ SUNDAY MEMORIAL SERVICE

      All Saints’ Sunday, in the traditional church calendar, provides the opportunity for the worshipping community to remember all persons of faith who have completed their walk in this life and now are in the immediate presence of their Lord.

      You are invited to remember family members, neighbors and friends with gratitude for their witness of faith and impact on our lives.  Remembering is a gift from God, which has an appropriate place in worship; the grief we continue to experience from the loss of loved ones also finds appropriate expression in the worship setting.

      New Member Information Form

      Completion of our Inquirer Class is required for membership in the Sturgeon Bay Moravian Church.  Upon completion of the class we ask that you fill out a membership form and return it to the church for the church records.

      Wedding Planning

      So, you’d like to be married in the Moravian Church, or by a Moravian Pastor.  Here are a few steps that may be of help to you as you prepare for your special day.

      • Contact the church office to set up an appointment with the pastor.
      • Make sure that the wedding day clears the pastor’s and church’s calendars.
      • Set up an appointment with the organist and wedding planner if you plan to be married in the church.
      • Stop in the church office and ask for a Wedding Planner booklet, ask that one be mailed to you or download the PDF copies below.  It contains information and examples which will help you with the planning of your wedding.
      • Complete the PDF version of the wedding planner information sheet and return it to the church office at: 323 South Fifth Avenue, Sturgeon Bay, WI 54235 or use the interactive form below - please note that this form contains 3 parts - Part 1 to be completed by both the bride and the groom and part 2 for the general information.
